Connecting Point IHSS - Nevada Sierra Plumas (Tri-Counties)
- Medical & Dental provided through Pan American Benefit Solutions. **Effective 6/1/24. These programs are FREE for all eligible providers who work and are paid 80 or more hours for two consecutive months. Application required.
- PPO Dental & Vision plan through MetLife. **Effective 6/1/24. All eligible providers who work and are paid 60 or more hours for two consecutive months.

UDW Member Benefits

Dental & Vision with MetLife - Voluntary HMO or PPO Dental Plan for All UDW members and/or family members. Enroll you or your family regardless of how many hours you are working. Complete the enrollment process yourself at https://udw.zevobenefits.com/ or call 800-883-0902 to enroll over the phone. Please note that any enrollment on the 20th of the month or before will be effective the 1st of the next month. If you are enrolled on the previous DHS HMO plan, please call DHS on or before the 15th of the month at 877-890-7023 to cancel your DHS individual plan as DHS will only accept that termination from the UDW / IHSS member.
- Single rates - HMO dental from $15.76, PPO dental from $34.83 and PPO vision from $4.67
